When an ovary is fixed to the uterus or another pelvic structure, <a href=\"https:\/\/nygse.com\/endometriosis-surgery-fertility-adhesion-barriers\/\"><strong>adhesions<\/strong><\/a> may be restricting its movement. It generally describes an ovary that has become fixed or adherent to nearby tissue rather than moving normally within the pelvis.<\/p><p>The ovary may become attached to structures such as the:<\/p><ul><li>Uterus<\/li><li>Pelvic sidewall<\/li><li>Fallopian tube<\/li><li>Bowel<\/li><li>Other surrounding pelvic tissues<\/li><\/ul><p>A study evaluating women with chronic pelvic pain or subfertility defined a fixed ovary on ultrasound as one that was adherent laterally to the pelvic sidewall or medially to the uterus. The ultrasound findings were then compared with findings during laparoscopy. <a href=\"#references\"><sup>[3]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0950e61 elementor-widget elementor-widget-heading\" data-id=\"0950e61\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Endometriosis Can Cause the Ovary to Adhere to Nearby Organs<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7145404 elementor-widget elementor-widget-text-editor\" data-id=\"7145404\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p><a href=\"https:\/\/nygse.com\/departments\/endometriosis\/\">Endometriosis<\/a> is associated with inflammation and can contribute to <strong>adhesion formation and fibrosis within the pelvis<\/strong>. Over time, adhesions can alter the normal relationships between pelvic organs.<\/p><p>An ovary may therefore become attached to the uterus or another nearby structure.<\/p><p>This can occur alongside an <a href=\"https:\/\/nygse.com\/endometrioma-surgery-fertility\/\"><strong>ovarian endometrioma<\/strong><\/a>, but an adherent ovary does not necessarily mean an endometrioma is present. Likewise, adhesions are not exclusive to endometriosis. Previous pelvic surgery, infection, and other inflammatory conditions can also contribute to adhesion formation.<\/p><p>For this reason, an ovary that appears stuck to the uterus can be an important clue, but <strong>it does not establish a diagnosis of endometriosis by itself<\/strong>.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94c45e elementor-widget elementor-widget-heading\" data-id=\"b94c45e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">What Symptoms Can an Adherent Ovary Cause?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eff36f4 elementor-widget elementor-widget-text-editor\" data-id=\"eff36f4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\tSome people with pelvic adhesions have no obvious symptoms.\n\nWhen symptoms are present, they may occur alongside other features associated with endometriosis, including:\n<ul>\n \t<li>Painful periods<\/li>\n \t<li>Chronic pelvic pain<\/li>\n \t<li>Pain during or after sex<\/li>\n \t<li>Painful bowel movements<\/li>\n \t<li>Pain at other points in the menstrual cycle<\/li>\n \t<li>Difficulty becoming pregnant<\/li>\n<\/ul>\nESHRE recommends considering endometriosis in patients with cyclical or noncyclical symptoms including dysmenorrhea, deep dyspareunia, dyschezia, urinary symptoms, fatigue, and infertility. <a href=\"#references\"><sup>[1]<\/sup><\/a>\n\nSymptoms alone, however, cannot determine whether an ovary is adherent or identify exactly where adhesions are located.\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-524637e elementor-widget elementor-widget-heading\" data-id=\"524637e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Ultrasound and MRI Can Help Identify Ovarian Adhesions<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9c443eb elementor-widget elementor-widget-text-editor\" data-id=\"9c443eb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>During a specialized transvaginal ultrasound, the clinician may assess whether an ovary <strong>moves freely or appears fixed to surrounding structures.<\/strong><\/p><p>In one study, ovarian fixation to the uterus on ultrasound showed <strong>89% sensitivity and 90% specificity for pelvic adhesions<\/strong> among women undergoing surgery for an endometrioma. <a href=\"#references\"><sup>[2]<\/sup><\/a><\/p><p>Ultrasound and MRI can help evaluate suspected endometriosis, although <strong>normal imaging does not completely exclude the condition<\/strong>. <a href=\"#references\"><sup>[1]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d37e14e elementor-widget elementor-widget-heading\" data-id=\"d37e14e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Can a Stuck Ovary Affect Fertility?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-83b0878 elementor-widget elementor-widget-text-editor\" data-id=\"83b0878\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Potentially, but a stuck ovary does <strong>not automatically mean infertility.<\/strong><\/p><p>Adhesions can distort the normal relationship between the ovary and fallopian tube, potentially interfering with the tube&#8217;s ability to capture the released egg. However, the effect on fertility varies from person to person.<\/p><p>Finding an ovary attached to the uterus therefore does not determine whether someone can become pregnant naturally.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b10ac51 elementor-widget elementor-widget-heading\" data-id=\"b10ac51\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Treatment Depends on Symptoms and Disease Extent<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f9fa4ba elementor-widget elementor-widget-text-editor\" data-id=\"f9fa4ba\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>A fixed ovary does <strong>not automatically mean surgery is necessary<\/strong>.
