The phrase may sound like a clot is growing inside the ovary, but medically, several different conditions can involve blood or clot-like material in or around an ovarian structure.<\/p><p>One possibility is an <a href=\"https:\/\/nygse.com\/endometrioma-surgery-fertility\/\"><strong>ovarian endometrioma<\/strong><\/a>, a cyst associated with endometriosis that contains blood products from repeated bleeding. Another is a <strong>hemorrhagic ovarian cyst<\/strong>, which forms when bleeding occurs within a functional ovarian cyst. This chronic hemorrhage contributes to the characteristic appearance of endometriomas on imaging. <a href=\"#references\"><sup>[3]<\/sup><\/a><\/p><p>This is also why endometriomas are sometimes informally called <strong>\u201cchocolate cysts\u201d<\/strong>. The term refers to their thick, old blood content rather than fresh bleeding.<\/p><p>Importantly, an endometrioma is not the same thing as a conventional blood clot circulating through a blood vessel.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2b9b067 elementor-widget elementor-widget-heading\" data-id=\"2b9b067\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Blood Clots Can Be Seen Within an Endometrioma<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b05102b elementor-widget elementor-widget-text-editor\" data-id=\"b05102b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Research has identified imaging findings that may represent <strong>chronic retractile blood clots within endometriomas.<\/strong><\/p><p>One example is the <strong>T2 dark spot sign<\/strong> on MRI. These dark spots are thought to represent chronic retracted blood clots within an endometrioma. Corwin et al. On ultrasound, fibrin strands and retracting clots may be visible. Because both hemorrhagic cysts and endometriomas contain blood products, their appearances can sometimes overlap. <a href=\"#references\"><sup>[4]<\/sup><\/a><\/p><p>MRI can provide additional information when ultrasound findings are uncertain. Endometriomas often demonstrate characteristic findings related to chronic blood products, including <strong>T2 shading<\/strong>, while other MRI characteristics can help distinguish them from hemorrhagic cysts. <a href=\"#references\"><sup>[4]<\/sup><\/a><\/p><p>The distinction matters because an endometrioma represents ovarian endometriosis, whereas a hemorrhagic functional cyst is a different ovarian condition.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94c45e elementor-widget elementor-widget-heading\" data-id=\"b94c45e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Ovarian Vein Thrombosis Is Different<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eff36f4 elementor-widget elementor-widget-text-editor\" data-id=\"eff36f4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>An <strong>ovarian vein thrombosis (OVT)<\/strong> is a true vascular blood clot that develops in an ovarian vein rather than within an endometrioma or ovarian cyst. <a href=\"#references\"><sup>[5]<\/sup><\/a><\/p><p>OVT is uncommon and belongs to the spectrum of venous thromboembolism. It has been reported particularly in association with pregnancy and the postpartum period, malignancy, pelvic infection, recent surgery, and other thrombotic risk factors. <a href=\"#references\"><sup>[5]<\/sup><\/a><\/p><p>This distinction is important:<\/p><ul><li><strong>Endometrioma:<\/strong> blood products accumulate within an ovarian cyst associated with endometriosis.<\/li><li><strong>Hemorrhagic ovarian cyst:<\/strong> bleeding occurs within an ovarian cyst and may produce a retracting clot.<\/li><li><strong>Ovarian vein thrombosis:<\/strong> a thrombus forms within the vein that drains the ovary.<\/li><\/ul><p>These conditions should not be described interchangeably.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-524637e elementor-widget elementor-widget-heading\" data-id=\"524637e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">What a \u201cBlood Clot in the Ovary\u201d May Mean<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-9c443eb elementor-widget elementor-widget-text-editor\" data-id=\"9c443eb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>If imaging,
