Birth control pills, progestins, and medications that suppress ovarian hormone production may reduce pain and menstrual symptoms. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>But hormones do not work equally well for everyone. Some patients continue to experience significant pain despite treatment. It can also be associated with <a href=\"https:\/\/nygse.com\/endometrioma-surgery-fertility\/\">endometriomas<\/a>, fibrosis, and adhesions that change normal pelvic anatomy. <a href=\"#references\"><sup>[2][3]<\/sup><\/a><\/p><p>Symptoms that continue despite treatment may include pelvic pain, painful periods, pain during sex, painful bowel movements, urinary symptoms, or pain that interferes with everyday activities. <a href=\"#references\"><sup>[2][4]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-2b9b067 elementor-widget elementor-widget-heading\" data-id=\"2b9b067\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Why Might Endometriosis Pain Continue Despite Hormones?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b05102b elementor-widget elementor-widget-text-editor\" data-id=\"b05102b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Hormonal medications can reduce the hormonal stimulation associated with endometriosis, but they do not surgically remove existing endometriosis lesions, endometriomas, fibrosis, or established adhesions. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>Pain can also have multiple causes. Pelvic floor dysfunction, <a href=\"https:\/\/nygse.com\/endometriosis-nerve-pain\/\">nerve-related pain<\/a>, and other gynecologic or gastrointestinal conditions may contribute to symptoms.<\/p><p>For this reason, persistent pain deserves further evaluation rather than automatically assuming that endometriosis is the only cause.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0950e61 elementor-widget elementor-widget-heading\" data-id=\"0950e61\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Other Options for Managing Endometriosis<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7145404 elementor-widget elementor-widget-text-editor\" data-id=\"7145404\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Depending on the patient&#8217;s symptoms and goals, treatment may include pain medication, pelvic floor physical therapy, other supportive pain-management strategies, another hormonal approach, or surgery. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>The appropriate next step depends on factors such as previous treatments, medication side effects, fertility plans, imaging findings, and how significantly symptoms affect quality of life.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94c45e elementor-widget elementor-widget-heading\" data-id=\"b94c45e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Should You Try Another Hormonal Treatment?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eff36f4 elementor-widget elementor-widget-text-editor\" data-id=\"eff36f4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Sometimes.<\/p><p>Different hormonal treatments work in different ways. A patient who did not improve with one medication may respond differently to another. <a href=\"#references\"><sup>[1]<\/sup><\/a><\/p><p>However, repeatedly changing medications is not necessarily the right approach for every patient. If symptoms remain significant, further evaluation can help determine whether another treatment strategy should be considered.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d61d104 elementor-widget elementor-widget-heading\" data-id=\"d61d104\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Surgery After Unsuccessful Hormonal Treatment<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5a37235 elementor-widget elementor-widget-text-editor\" data-id=\"5a37235\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Surgery is another treatment option for endometriosis-associated pain. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>A surgical evaluation may become particularly relevant when symptoms remain significant despite medical treatment or when imaging or examination suggests findings such as:<\/p><ul><li><a href=\"https:\/\/nygse.com\/departments\/ovarian-endometrioma\/\">Ovarian endometriomas<\/a><\/li><li><a href=\"https:\/\/nygse.com\/departments\/deep-infiltrating-endometriosis-die\/\">Deep endometriosis<\/a><\/li><li><a href=\"https:\/\/nygse.com\/departments\/pelvic-adhesions\/\">Significant pelvic adhesions<\/a><\/li><li>Distorted pelvic anatomy<\/li><li>Suspected bowel, bladder, or ureter involvement<\/li><\/ul><p>The presence of these findings does not automatically mean surgery is necessary. The potential benefits and risks should be considered for each patient. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0135b56 elementor-widget elementor-widget-heading\" data-id=\"0135b56\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Does Hormonal Treatment Have to Fail Before Surgery?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e8fd448 elementor-widget elementor-widget-text-editor\" data-id=\"e8fd448\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Not necessarily.<\/p><p>There is no requirement that every patient must try multiple hormonal medications before discussing surgery. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>Some patients cannot tolerate hormonal treatment. Others may have medical reasons for avoiding certain medications, be trying to become pregnant, or have symptoms and clinical findings that warrant a surgical evaluation.<\/p><p>Treatment decisions should consider the patient&#8217;s preferences as well as symptoms, previous treatment, fertility goals, and suspected extent of disease. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e4fb430 elementor-widget elementor-widget-heading\" data-id=\"e4fb430\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Endometriosis Excision Surgery<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-860653e elementor-widget elementor-widget-text-editor\" data-id=\"860653e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>When surgery is appropriate, <strong>excision surgery<\/strong> involves identifying and surgically removing areas of endometriosis. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>The complexity of the procedure can vary considerably. Disease limited to certain pelvic areas may require a different surgical approach than deep endometriosis involving the bowel, bladder, ureters, ovaries, or extensive adhesions.<\/p><p>Surgery also has potential risks and does not guarantee that pain will completely disappear or never return.
