These ligaments are located deep within the pelvis and help support the uterus and cervix.<\/p><p>They are also an important location for <a href=\"https:\/\/nygse.com\/departments\/deep-infiltrating-endometriosis-die\/\"><strong>deep endometriosis<\/strong><\/a>. Current European Society of Urogenital Radiology (ESUR) recommendations consider a uterosacral ligament abnormal when a nodule or spiculation is visible in at least two imaging planes or when a characteristic bright T1-weighted focus is present. A <em>radiologist may describe findings<\/em> such as:<\/p><ul><li>Uterosacral ligament thickening<\/li><li>Nodular thickening<\/li><li>Fibrotic thickening<\/li><li>Spiculation<\/li><li>A discrete uterosacral ligament nodule<\/li><\/ul><p>The location can also matter. MRI reports may specify whether the finding involves the right or left ligament and whether it is closer to the cervix or extends farther into the posterior pelvis. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94c45e elementor-widget elementor-widget-heading\" data-id=\"b94c45e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Does Thickening Always Mean Endometriosis?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eff36f4 elementor-widget elementor-widget-text-editor\" data-id=\"eff36f4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>No.<\/p><p><strong>Uterosacral ligament thickening by itself does not confirm endometriosis<\/strong>. Imaging findings need to be interpreted together with symptoms, examination findings, other areas seen on MRI or ultrasound, and the patient&#8217;s medical history. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>This distinction is important because even expert MRI criteria do not consider ligament thickness alone specific enough to diagnose an endometriotic deposit. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d61d104 elementor-widget elementor-widget-heading\" data-id=\"d61d104\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Symptoms Associated With Uterosacral Ligament Endometriosis<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5a37235 elementor-widget elementor-widget-text-editor\" data-id=\"5a37235\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Some patients may have no symptom that points specifically to the uterosacral ligaments.<\/p><p>When symptoms are present, they may occur as part of a broader pattern of deep endometriosis and can include pelvic pain, painful periods, deep pain during sex, or pain that extends toward the lower back or posterior pelvis. <a href=\"#references\"><sup>[4]<\/sup><\/a><\/p><p>Symptoms alone cannot determine exactly where endometriosis is located.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0135b56 elementor-widget elementor-widget-heading\" data-id=\"0135b56\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Can MRI Help Before Endometriosis Surgery?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e8fd448 elementor-widget elementor-widget-text-editor\" data-id=\"e8fd448\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Yes. MRI can be particularly useful for <strong>mapping deep endometriosis before surgery.<\/strong> <a href=\"#references\"><sup>[3]<\/sup><\/a><\/p><p>Knowing that disease may involve the uterosacral ligaments and nearby structures can help the surgical team understand the distribution of suspected disease and plan an appropriate approach. Current ESUR recommendations specifically support MRI in patients being considered for surgical or interventional <a href=\"https:\/\/nygse.com\/treatments\/\">treatment of endometriosis<\/a>.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e4fb430 elementor-widget elementor-widget-heading\" data-id=\"e4fb430\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">When Uterosacral Ligament Thickening Deserves Further Evaluation<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-860653e elementor-widget elementor-widget-text-editor\" data-id=\"860653e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>A finding of uterosacral ligament thickening or nodularity does not automatically mean that surgery is necessary.<\/p><p>However, when it appears alongside persistent pelvic pain, painful periods, deep pain during sex, endometriomas, <a href=\"https:\/\/nygse.com\/departments\/bowel-endometriosis\/\">bowel symptoms<\/a>, or other MRI findings associated with deep endometriosis, further evaluation may be appropriate.<\/p><p>At <strong>New York Gynecology Surgery &amp; Endometriosis (NYGSE)<\/strong>, we consider the complete pattern of symptoms and imaging findings to determine whether deep endometriosis may be contributing
