When endometriomas are found in both ovaries, they are called <strong>bilateral endometriomas<\/strong>. <a href=\"#references\"><sup>[2]<\/sup><\/a><\/p><p>Endometriomas are ovarian cysts associated with <a href=\"https:\/\/nygse.com\/departments\/endometriosis\/\">endometriosis<\/a>. They typically contain old blood and are sometimes called <strong>\u201cchocolate cysts\u201d<\/strong> because of their appearance.<\/p><p>When both ovaries are affected, careful evaluation is particularly important. Age, the size and number of endometriomas, previous ovarian surgery, and other individual factors may also influence ovarian function. <a href=\"#references\"><sup>[2][4]<\/sup><\/a><\/p><p>When both ovaries are affected, preserving healthy ovarian tissue can become especially important, particularly for patients who hope to become pregnant in the future. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>For patients who undergo endometriosis surgery, the <a href=\"https:\/\/nygse.com\/endometriosis-fertility-index\/\">Endometriosis Fertility Index (EFI)<\/a> is a scoring system used to help assess fertility prognosis after surgery.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b94c45e elementor-widget elementor-widget-heading\" data-id=\"b94c45e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Can You Get Pregnant With Bilateral Endometriomas?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eff36f4 elementor-widget elementor-widget-text-editor\" data-id=\"eff36f4\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Yes. <strong>Having bilateral endometriomas does not automatically mean that you cannot become pregnant.<\/strong><\/p><p>Fertility depends on much more than the presence of ovarian cysts. Doctors may consider age, ovarian reserve, fallopian tube function, pelvic adhesions, other areas of endometriosis, previous surgery, and sperm factors.<\/p><p>Some patients with endometriomas conceive naturally, while others may benefit from fertility treatment. Surgery for an endometrioma can sometimes affect healthy ovarian tissue and may reduce ovarian reserve. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p><p>The potential benefit of removing an endometrioma therefore needs to be weighed against the potential effect of surgery on ovarian function. <a href=\"#references\"><sup>[1][2]<\/sup><\/a><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-0135b56 elementor-widget elementor-widget-heading\" data-id=\"0135b56\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">What If Both Endometriomas Are Large?<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e8fd448 elementor-widget elementor-widget-text-editor\" data-id=\"e8fd448\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Cyst size is an important consideration, but size alone does not determine whether surgery is necessary. <a href=\"#references\"><sup>[2]<\/sup><\/a><\/p><p>Doctors may also consider whether an endometrioma is causing significant symptoms, changing over time, has unusual imaging characteristics, affects fertility treatment, or occurs alongside other forms of endometriosis. <a href=\"#references\"><sup>[2]<\/sup><\/a><\/p><p>For this reason, two patients with similarly sized bilateral endometriomas may receive different treatment recommendations based on their individual circumstances.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-e4fb430 elementor-widget elementor-widget-heading\" data-id=\"e4fb430\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Bilateral Endometriomas and More Extensive Endometriosis<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-860653e elementor-widget elementor-widget-text-editor\" data-id=\"860653e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Endometriomas can occur alongside pelvic adhesions and deep endometriosis. <a href=\"#references\"><sup>[3]<\/sup><\/a><\/p><p><a href=\"https:\/\/nygse.com\/adhesion-barriers-endometriosis-surgery\/\">Adhesions<\/a> may restrict normal ovarian movement or cause the ovaries to become attached to surrounding structures. In some patients, both ovaries are pulled toward the center of the pelvis and positioned next to one another behind the uterus. This imaging finding is sometimes called <a href=\"https:\/\/nygse.com\/kissing-ovaries-ultrasound-mri\/\">kissing ovaries<\/a>. <a href=\"#references\"><sup>[3]<\/sup><\/a><\/p><p>Looking at the position and mobility of the ovaries, as well as the surrounding pelvic anatomy, can therefore provide information beyond the endometriomas themselves.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-eb8aec8 elementor-widget elementor-widget-heading\" data-id=\"eb8aec8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">Planning Treatment for Bilateral Endometriomas<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-005d958 elementor-widget elementor-widget-text-editor\" data-id=\"005d958\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t\t\t\t\t\t<p>Bilateral endometriomas are an important finding in ovarian endometriosis, but the cysts alone do not determine what treatment a patient needs.<\/p><p>Further evaluation may be particularly important for patients experiencing pelvic pain, painful periods, fertility concerns, changes in cyst size, or other symptoms associated with
